Monarch Support services

Monarch Support Services is committed to serving individuals in the disability community by establishing a pathway to overcome barriers. We’re an approved Home and Community-Based Services (HCBS) provider.

Monarch Support Service is a Home and Community-Based Services (HCBS) licensed provider under Minnesota Chapter 245D. Monarch Support Services is offering 24-Hour Emergency Assistance. Through person centered planning we help individuals achieve the change they want and assist them to reach their full potential of long-term stability to sustain their desired quality of life. 

Person-Centered 24-Hour Emergency Assistance services are designed to provide individuals with immediate access to support during emergencies or unexpected situations that may impact their health, safety, or ability to remain stable in their home and community. Services are delivered in a manner that respects each person’s unique needs, preferences, strengths, and goals, while promoting independence and self-determination.

Support is individualized and based on the person’s coordinated service and support plan and any applicable addenda. The individual directs how and when emergency assistance is accessed, identifies preferred methods of contact, and helps determine what situations constitute an emergency for them. Services focus on stabilization, de-escalation, problem-solving, and connection to appropriate natural supports, community resources, or emergency services when necessary.

24-Hour Emergency Assistance emphasizes consistency, trust, and clear communication. Individuals have access to reliable on-call support outside of regularly scheduled services to help manage crises related to mental health, safety concerns, housing stability, or other urgent needs. The service is not a replacement for medical or law-enforcement intervention but works collaboratively with those systems when required.

The goal of person-centered 24-Hour Emergency Assistance is to enhance safety, reduce unnecessary hospitalization or displacement, and support individuals in maintaining stability and independence in their own homes and communities while honoring their choices, dignity, and autonomy.
